Important Documents & Forms

This website provides general information to help you choose and use your PNC benefits. If you’d like additional details, or information on specific situations or requirements, it’s important that you review the list of documents below.

PNC Benefits Information

Below are the current benefit booklets — known as Summary Plan Descriptions (SPDs) — for each PNC benefit. An SPD is your key resource to understand how the plan works, including coverage, eligibility, enrollment, how to file claims/appeals and your legal rights.

In some cases, there is a Summary of Material Modifications (SMM) listed under the SPD. An SMM updates and supplements the applicable SPD, and both documents should be reviewed together for the most accurate and complete information.

Health Savings Account (HSA)

While employee pretax HSA payroll contributions are made under the terms of the PNC Group Benefit Plan, the HSA itself is not part of the PNC Group Benefit Plan, and is not covered by the Employee Retirement Income Security Act of 1974, as amended (ERISA). It is offered by PNC Bank, National Association, independent of the PNC-sponsored employee benefit programs. Although PNC intends to continue its contribution to the HSA, it reserves the right to change, modify, amend or terminate this at any time and for any reason.

Transparency in Coverage

PNC and most other group health plans are required to publicly post pricing information for certain covered items and services. This pricing information must be in machine-readable format and can be used by third parties, such as researchers and app developers to help consumers better understand the costs associated with their health care. To access machine-readable files from PNC’s three medical carriers, please use the links below.
